While the birth registration doesn't say so, the family was probably in or near the village of Sheffield, Ontario, at the time.

L. Merl Gamble Koepel, age 21, single, died April 12, 1914, at 8 Irene Ave. Informant B. D. Humphrey, 1058 Yonge St. Cause of death pulmonary tuberculosis.

The informant, B. D. Humphrey, was the undertaker. Myrtle, or Merl as she seems to have been known, was buried in Mt. Pleasant Cemetery, Toronto, and the record is to be found in the burial record pages of the Toronto Trust Cemeteries, which is a joint project of the Toronto Branch of the Ontario Genealogical Society and Familysearch. The burial record, as with Merl's death registration, shows her as single. The burial plot, or perhaps simply a grave, was purchased by Albert E. Churchill of 8 Irene Ave. The minister who conducted the burial service was a C. James. A newspaper death notice or obituary has not been found.
